I remember reading in one of the item descriptions that she is the only needle knight that remains because of this. I think one of the talismans you get for helping her kill hornsent or ansbach. She's always been the paranoid type and miquella probably enchanted her to subdue this tendency of hers.

The fact that her paranoia only starts when miquella's charm is broken kinda gives up a glimpse on the kind of world miquella would've brought over as elden lord.
His new order would ensure devotion by stealing the hearts of others and if they ever resist the charm or break free they would be executed out of fear of no longer being loyal to miquella, with the lord himself trying to kill/charm the tarnished instead of trying to talk to him and convince him that his way is better than the old order.
