magicbufo4277: I would also put Monk's Flameblade and Watchdogs Greatsword as honourable mentions, took me quite a few runs.
jettblade: Monk's Flameblade has only a single enemy that drops and it that isn't very close to the grace so you'll have to trek to it, also is in a fairly tough area. It does have a 4% drop rate so its much higher than some other drops but the difference is there's only one place to get it.
Watchdog's Greatsword is another one where there are only a few places to get it but you have to beat some tough enemies. Watchdogs are pretty strong in the Mountaintop of the Giants and can be hard to deal with. It does have an 8% drop chance so its not too bad.
Iron Greatsword is dropped from two Red-maned Misbegottens which can be tough. One is in Leyndell tucked in a corner a pretty far distance from a grace but there is one in the Halitree that is really close. They have a 4% drop rate.
Other rare drops are: Iron Spear, Claymen Harpoon, Rotten Crystal Spear, Rotten Crystal Staff, Magma Whip, and Golem Fist. All have a pretty low drop chance or on really tough enemies.

willb6913: Warhawk's Talon from the murder birds in Stormveil Castle. They have a very cool moveset with a unique charged heavy attack, they look great and they don't even weigh that much.
